REQUIREMENTS FOR SECURING A MAYOR’S PERMIT ON BUSINESS IN TAGBILARAN CITY: |
| 1. |
BARANGAY CLEARANCE - from the Barangay where business
establishment or activity is located; |
| 2. |
TAX CLEARANCE - from the City Treasurer’s Office
showing that the operator has paid all tax obligations in the City; |
| 3. |
COMMUNITY TAX - currently issued at the City Treasurer’s
Office and deputized Barangay Treasurers; |
| 4. |
PHOTOS/PICTURES - 1pc. passport size
- for single proprietorship, picture of owner/proprietor
- for corporations/partnership, picture of the General Manager or Managing Partner. |
| 5. |
PROPERTY IDENTIFICATION NUMBER (PIN) of the location
of the business establishment - to be obtained at the City Assessor's Office. |
| 6. |
Duly accomplished AFFIDAVIT OF CAPITAL - form can
be obtained from the City Treasurer's Office, or can be downloaded here. |

OTHER REQUIREMENTS FOR THE SPECIFIED BUSINESS: |

LEASE CONTRACT - All vendors at the City Public
Markets (Cogon, Manga, Fish Port Terminal, City Central Market and Integrated
Bus Terminal.) |
| > |
REAL ESTATE LESSOR'S PERMIT - (Xerox Copy) of the
owner of the building or lot rented - for those business permit applicants who
are renting, leasing a space, building or lot. |
| > |
CERTIFICATE OF OCCUPANCY - (Xerox Copy) if building
is Newly Constructed/Renovated - from the City Engineer's Office |
| > |
ANNUAL CERTIFICATE OF INSPECTION OF BUSINESS ESTABLISHMENT-
from City Engineer's Office |

REQUIREMENTS FOR EMPLOYEES OF THE BUSINESS ESTABLISHMENTS: |
| 1. |
Barangay Clearance obtained from where the business
establishment is located. |
| 2. |
Community Tax Certificate - obtained from the City
Treasurer's Office or from the barangay where the business establishment is located. |
| 3. |
Police Clearance - obtained from the City Police
Station.
(note: the following are needed before a police clearance can be issued: barangay
clearance and community tax certificate) |
| 4. |
Health Clearance/Certificate - from the City Health
Office. |
|
NOTE: New Applicants for Business Permit will be entertained beginning February until
December. |
